Release of Liabilities
Members sign this Release of Liabilities as part of the online membership form on Zeffy when they join.
Guests sign it on the paper sign-in sheet at the door every time they attend. There is no online Release of Liabilities for guests.

“The Swing Dance Club of Hawaii and the Department of Parks and Recreation of the City of Honolulu are permitting and allowing me to participate in activities at the City and County of Honolulu facility aka The Ala Wai Golf Multipurpose Facility aka The Palladium.”
“For and in consideration of the foregoing, I/we hereby release and forever discharge and promise not to hold liable the City and County of Honolulu Parks Board, Swing Dance Club of Hawaii, respective officers, and Board Members from any action, suits, damages, claims or judgments that may result from any personal injury or loss of property.”
“This release is binding to me/us and my/our heirs and personal representatives and is valid for all Swing Dance Club of Hawaii activities that I participate in.”
